Key Responsibilities- Lead and manage the Community Support Team, providing supervision, support, and evaluation of team members.
- Ensure that all team members are effectively coordinating care and delivering services that meet the unique needs of each client.
- Oversee the development and implementation of person-centered treatment plans, ensuring that they are aligned with the client's goals and recovery journey.
- Conduct assessments, provide direct services, and intervene in crises when necessary.
- Monitor and ensure compliance with all state, federal, and agency regulations and policies.
- Facilitate regular team meetings and case conferences to promote collaboration and information sharing.
- Provide ongoing training and professional development opportunities for team members.
- Collaborate with external partners, including other healthcare providers, social services, and community organizations, to enhance service delivery and client outcomes.
- Collect and analyze data to monitor the effectiveness of the program and make recommendations for improvement.
- Advocate for clients within various systems, including healthcare, judicial, and social services.
- A Master's degree in social work, counseling, psychology, or a related field.
- Licensure within the state of North Carolina (LCSW(A), LCMHC(A) LMFT).
- A minimum of 1 year of experience in mental health or substance use services.
- Strong knowledge of evidence-based practices in mental health and substance use treatment.
- Excellent leadership, organizational, and communication skills.
- A commitment to person-centered care and cultural competence.
- A valid driver's license and reliable transportation.
