Job Description\
As a Community Health Program Coordinator, you will conduct outreach and engagement activities, develop person-centered treatment plans, and collaborate with care teams to deliver high-quality services. You will also ensure seamless care transitions, provide health and wellness coaching, and connect enrollees with needed services and supports.\\/p>Requirements\
To be successful in this role, you will need a BA/BS degree in a field related to human services or a High School diploma/GED with 3 years of experience working with adults in community-based and/or medical settings. You must have excellent verbal and written communication skills, strong computer proficiency, and the ability to maintain personal and professional boundaries.\\/p>Benefits\
The salary for this position is $35/hour. As a full-time employee, you can accrue up to 3 weeks of vacation and 2 weeks of sick time. You will also be eligible for paid holidays, employer-paid life insurance & AD&D, long-term disability, comprehensive benefits packages, and retirement plans.\\/p>
