We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN in Bloomington. As a Home Health Registered Nurse RN, you will play a vital role in providing high-quality patient care and education to patients and their families in the comfort of their own homes.
Responsibilities:- Provide skilled nursing visits to patients in their homes, including admission, case management, and follow-up visits.
- Develop and implement individualized plans of care based on patient assessment findings and physician orders.
- Confer with physicians and patients to develop and update plans of care.
- Train and educate patients and their families on disease conditions and treatment plans.
- Coordinate with other healthcare disciplines to ensure continuity of care.
- Update physicians on patient status and provide routine updates as needed.
- Work with families to involve them in the plan of care and achieve maximum resolution to patient needs and problems.
- Supervise home health aides and ensure they are following the plan of care.
- Perform discharge planning and provide patients with necessary information for continued care after home health services are discontinued.
- Associate's degree in Nursing.
- Valid RN license in the state of employment.
- Valid driver's license.
- Able and willing to travel.
- Ability to successfully complete required background check and drug screen.
- Excellent writing and charting skills, experience with electronic medical records preferred.
