Position Overview:
This is a full-time role with a structured schedule of Monday to Friday, totaling 40 hours weekly. Enjoy a work-life balance with no weekend, night, or holiday shifts.
Key Responsibilities:
- Deliver patient care at designated facilities during agreed-upon times.
- Collaborate with facility staff to optimize patient workflow.
- Proactively monitor and treat patients to prevent unnecessary hospitalizations.
- Engage with attending physicians to oversee medication management.
- Manage patient conditions such as weight loss, pain, wound care, and the administration of antibiotics and IVs.
- Conduct thorough patient assessments to ensure high-quality care.
- Communicate effectively with patients' families, including monthly updates on patient status.
- Address advanced care directives when suitable.
- Collaborate closely with TapestryHealth's clinical team and specialty clinicians.
Documentation:
Complete all patient notes within 48 hours of the visit.
Qualifications:
- Must hold a valid nursing license in Pennsylvania.
- Prior experience with the geriatric population is essential.
- Strong assessment and communication abilities are required.
- Familiarity with medication management and de-escalation techniques is necessary.
- Proof of COVID vaccination is mandatory.
- Experience in ICU, ER, Urgent Care, Primary Care, or Surgical settings will be advantageous.
Compensation and Benefits:
Along with a competitive salary, you will have access to incentive compensation. We offer a comprehensive benefits package, including 4 weeks of paid time off, CME time off, an annual CME allowance, and malpractice insurance.
