About the Role:
The University of Rochester is seeking a skilled and compassionate RN Coordinator to join our team. This exciting opportunity allows the right candidate to make a meaningful impact in the lives of our patients.
Job Summary:
We are looking for a motivated and self-directed RN to coordinate complex care, provide patient education, and advocate on behalf of our patients. As a key member of our interdisciplinary team, you will work closely with RNs, Surgeons, and other healthcare professionals to ensure seamless patient care and navigation through the healthcare system.
Key Responsibilities:
- Care Plan Development: Collaborate with internal and external teams to develop and implement effective care plans tailored to each patient's unique needs.
- Patient Education and Counseling: Provide comprehensive education and counseling to patients and their families, ensuring they are informed decision-makers throughout the care process.
- Patient Advocacy: Act as a passionate advocate for our patients, communicating their needs and preferences to healthcare professionals and navigating the healthcare system on their behalf.
- Interdisciplinary Collaboration: Work collaboratively with RNs, Surgeons, and other healthcare professionals to address complex care needs and ensure continuity of care.
- Effective Communication and Documentation: Ensure accurate and timely communication and documentation to support seamless patient care and coordination.

Compensation and Benefits:
We offer a competitive salary range of $70,000 - $90,000 per year, commensurate with experience, along with a comprehensive benefits package that includes health insurance, retirement savings plan, and generous paid time off.
