Job Summary

We are seeking a highly skilled Infectious Disease Physician to join our team at AbsoluteCare, Inc. As a key member of our multidisciplinary care team, you will provide comprehensive care to patients living with HIV and other chronic infectious diseases.

Key Responsibilities
  • Provide patient consultation, primary management, and ongoing treatment of chronic infectious diseases.
  • Develop diagnostic and treatment care plans, educate patients about their illnesses, and advise on specialist services.
  • Work collaboratively with a large, integrated care team to provide whole-person care in the outpatient setting.
  • Implement plans of care in collaboration with patients, their families, and other team members.
  • Develop strong transitional care plans and track patients along the continuum of care, including hospital discharges, ER visits, and readmissions.
Requirements
  • Unrestricted Pennsylvania Medical License
  • Board certification in Internal Medicine, Family Practice, or Infectious Diseases
  • Experience in population health management and working with psychosocially and medically complex patients
  • Knowledge of the NCQA PCMH model and ability to work as part of a multidisciplinary team
