Job Summary

We are seeking a highly skilled Infectious Disease Physician to join our team at AbsoluteCare. As a key member of our multidisciplinary team, you will provide comprehensive care to patients living with HIV and other chronic infectious diseases.

Key Responsibilities
  • Provide patient consultation and primary management of HIV and other chronic infectious diseases.
  • Develop and implement diagnostic and treatment care plans, counseling patients on their illnesses and advising on specialist services.
  • Work collaboratively with a large, integrated care team to provide whole-person care in the outpatient setting.
  • Manage chronic diseases directly or through collaboration with primary care physicians and specialists.
  • Implement plans of care in collaboration with patients, their families, and other team members.
  • Develop strong transitional care plans and track patients along the continuum of care.
  • Prevent ER visits, admissions to the hospital, and readmissions through vigilant tracking of patients and their disease processes.
Requirements
  • Unrestricted Maryland Medical License
  • Board certified in Internal Medicine or Family Practice
  • Certified by AAHIVM or Board Certification in Infectious Diseases
  • BLS and DEA certification
  • Experience in population health management and working with psychosocially and medically complex patients
  • Knowledge of the NCQA PCMH model
  • Excellent interpersonal and communication skills
  • Ability to work as part of a multidisciplinary team and adapt to a fast-paced environment
Working Conditions

This role operates in a patient care setting where there is potential exposure to blood, bodily fluids, and infectious materials. The job may be physically demanding at times, requiring the ability to remain stationary for extended periods and perform repetitive movements.
