Job Title: Experienced Speech Therapist

Human Touch Home Health is seeking an experienced speech therapist to join our dynamic team. As a speech therapist, you will be responsible for providing expert speech therapy services to patients in their homes.

Job Summary:

We are looking for a skilled speech therapist to work with our interdisciplinary team to provide comprehensive speech therapy services to patients. The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to work independently.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical and functional abilities.
  • Develop personalized treatment plans in accordance with physician recommendations and patients' goals.
  • Provide skilled speech therapy interventions aimed at improving mobility, strength, balance, and overall function.
  • Educate patients and their caregivers on therapeutic exercises, safety precautions, and injury prevention strategies.
  • Monitor patients' progress and adjust treatment plans as necessary to optimize outcomes.
  • Document all clinical findings, interventions, and patient responses accurately and in a timely manner.
  • Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care and achieve desired outcomes.
  • Maintain compliance with regulatory standards, including HIPAA and Medicare guidelines.
  • Participate in ongoing professional development activities to enhance clinical skills and stay abreast of best practices in home health speech therapy.
Requirements:
  • Graduate of an accredited speech therapy program with a current license to practice in the state which you applied.
  • Minimum of 2 years of clinical experience as a speech therapist, with a focus on home health care or skilled nursing facility preferred.
  • Strong clinical reasoning and problem-solving skills, with the ability to adapt treatment plans based on individual patient needs.
  • Strong clinical skills in assessment, treatment planning, and therapeutic interventions for a diverse patient population, including adults and older adults.
  • Confidence in recognizing medical emergencies and responding appropriately.
  • Excellent interpersonal and communication skills, with the ability to effectively collaborate with patients, families, and interdisciplinary team members.
  • Demonstrated proficiency in conducting comprehensive speech therapy evaluations and developing evidence-based treatment plans.
  • Ability to work independently, manage time effectively, and prioritize tasks in a fast-paced environment.
  • Proficiency in using electronic medical records (EMR) systems for documentation purposes. Kinnser Well sky or Homecare Homebase preferred.
  • Experience with oasis preferred.
  • Reliable transportation and a valid driver's license required.
  • BLS certification and other relevant certifications may be required.
  • Recent TB screen or chest x-ray.
