We are seeking a highly skilled and compassionate Care Coordinator to join our team at Hoag Memorial Hospital Presbyterian. As a key member of our care team, you will play a vital role in delivering exceptional patient experiences and ensuring seamless operational workflows.
Key Responsibilities- Provide administrative support to clinicians and health coaches, including chart preparation, scheduling, and appointment follow-up tasks.
- Schedule and conduct welcome calls with new patients to onboard them to our care program and schedule their first round of appointments.
- Assist with daily practice operations, including answering inbound phone calls, insurance and ID verification, and managing in-app messages.
- Collaborate with clinical teams to resolve administrative member requests and overflow tasks, including medical record requests and coordination.
- Provide practice-wide messaging and phone coverage, triaging time-sensitive messages and escalating to management as needed.
- 3+ years of experience in healthcare, care coordination, patient experience, or member concierge.
- Bachelor's degree or equivalent preferred.
- Proven experience at an innovative healthcare company, preferably a membership-based company where technology was used to facilitate care.
