Job Summary:This is a 32-hour per week contract position with 13 weeks guaranteed. The ideal candidate will have experience working in a Skilled Nursing Facility (SNF) and be comfortable with the requirements of a travel assignment.
Responsibilities:- Provide physical therapy services to patients in a SNF setting
- Conduct initial assessments and develop individualized treatment plans
- Implement and modify treatment plans as needed
- Communicate effectively with patients, families, and healthcare team members
- Meet productivity and documentation requirements
- Current Physical Therapy license in the state of practice
- Experience working in a SNF setting
- Ability to pass a background check and drug screen
- Current CPR certification
