Your Role

We are seeking a highly skilled and compassionate Healthcare Solutions Specialist to join our dedicated team. As a key member of our care management team, you will be responsible for collaborating with physicians, advanced practice providers, and ancillary team members to manage patients' specialized needs.

Your primary focus will be on patients with complex chronic conditions, serious illness, or advanced frailty. You will perform comprehensive assessments to identify patients' needs, develop personalized care management plans, and coordinate care delivery with our clinicians, network providers, and community-based services.

Key Responsibilities
  • Build empathetic relationships with patients and families to understand their unique needs and preferences.
  • Evaluate and identify patients' clinical needs based on their values, goals, and circumstances.
  • Develop and implement comprehensive care management plans in collaboration with our multidisciplinary team.
  • C-coordinate access to healthcare services across multiple providers and disciplines.
  • M-onitor patient progress, identify areas for improvement, and modify care plans as needed.
  • R-refer patients to internal resources and external partners for additional support and services.
Requirements

To be successful in this role, you will need:

  • A current Registered Nurse license, with a Bachelor's of Science in Nursing preferred.
  • 3+ years of clinical experience in a hospital, clinic, home care, or nursing home setting.
  • Case management experience with a senior population, disease management, and/or physician office experience highly preferred.
  • Prior experience with electronic health records or health registries required.
  • Microsoft Office skills, including Excel, Word, PowerPoint, Outlook, required.
What We Offer

We offer a competitive salary range of $76,660 - $88,000 per year, depending on your experience and qualifications. In addition, we provide a comprehensive benefits package, including paid time off, 12 paid holidays, reimbursement for continuing medical education, 401k with match, health, dental, and vision insurance, and family-friendly policies that support paid parental leave and flexible work arrangements.
