**Responsibilities:**
- Conduct thorough social services evaluations and assessments to identify residents' needs and develop personalized care plans.
- Provide counseling services, referrals, and support to residents and their families, addressing issues such as financial and legal matters, discharge planning, and psychiatric medication management.
- Collaborate with healthcare professionals, families, and community agencies to ensure seamless transitions and optimal care outcomes.

**Requirements:**
