**Job Summary**
This role requires a strong commitment to delivering exceptional support services to our residents. You will be responsible for conducting periodic assessments, advocating on behalf of residents, participating in health care integration initiatives, responding to emergency calls, educating residents about available services, pursuing additional services through various sources, documenting resident contacts, and coordinating care across medical providers.
**Key Responsibilities**
- Conduct regular assessments of residents' social, psychological, cognitive, and physical status and develop tailored plans to address their needs.
- Advocate effectively on behalf of residents to ensure timely and cost-effective access to essential services.
- Pursue innovative solutions to enhance the overall well-being of our residents.
- Maintain accurate and confidential records of significant contacts with residents, service providers, families, and other stakeholders.
**Requirements**
To succeed in this role, you will need:
- A Master's or Bachelor's degree in Social Work, Gerontology, or a related field.
- Resident Services Coordinator Certification or willingness to attain it within the first year of employment.
- Proven experience working with older adults.
- Proficiency in Microsoft Office products and database software.
- Bilingualism in Russian (a plus).
**What We Offer**
In return for your expertise and dedication, we offer a competitive salary range of $55,000 - $65,000 per annum, depending on experience. Additionally, you will enjoy a rich benefits package, including a 27% increase on base pay, a robust medical and dental plan, employer-paid life insurance, a 401(k) plan, paid vacation, paid holidays, and more.
