Chronic Care Management Services (CCM)

Chronic Care Management (CCM) represents a healthcare delivery model that aims to provide thorough and coordinated care for patients with chronic health conditions. Our CCM services are designed to enhance the quality of care, improve patient outcomes, and lower healthcare expenses by addressing the intricate needs of individuals living with chronic illnesses.

Responsibilities of the Care Coordinator:

Crafting individualized care plans in partnership with patients, caregivers, and healthcare professionals. Facilitating and coordinating communication among various members of the healthcare team, including physicians, specialists, nurses, and ancillary service providers. Overseeing patients' adherence to treatment protocols and medications, along with their progress towards health objectives. Offering continuous education and support to patients and their families regarding disease management, lifestyle changes, and preventive care strategies. Employing electronic health records (EHR) and other healthcare technology tools to document patient interactions, monitor interventions, and ensure seamless continuity of care. Collaborating with community resources and social services agencies to address patients' social determinants of health and enhance their overall well-being. Conducting regular follow-up check-ins and care coordination activities to optimize health outcomes and improve the quality of life for patients with chronic conditions.

Qualifications for the Care Coordinator:

Minimum of 1 year of clinical experience as a Medical Assistant. At least 1 year of experience with electronic medical records; eClinicalWorks experience preferred. High degree of professionalism. Exceptional written and verbal communication abilities. Strong customer service orientation. Completion of a Medical Assistant Course or possession of a CMA designation is required.
