The Care Navigator plays a vital role in guiding patients and their families through the healthcare system. This position requires strong communication and interpersonal skills to assist patients with access issues, develop relationships with service providers, and connect them with community resources.
Key Responsibilities- Assist patients and families with prescriptions, travel, and access to financial resources
- Develop relationships with service providers and community resources
- Provide information on insurance benefits and co-pays
- Guide patients through state and federal programs
- Answer questions regarding patient insurance benefits
- Associate's degree in health sciences and/or medical records certificate program preferred
- Minimum of two years of experience in the healthcare field preferred
- Basic Life Support (BLS) certification
