Job Summary

We are seeking a highly skilled and compassionate Sr. CCBHC Case Manager to join our team at Outreach Development Corp. As a key member of our staff, you will play a vital role in providing case management and crisis intervention services to individuals in need.

Key Responsibilities
  • Case Management: Coordinate treatment services, promote access to transitional support services, and assist clients with barriers to employment and self-sufficiency.
  • Crisis Intervention: Provide immediate support and guidance to clients in crisis, ensuring their safety and well-being.
  • Advocacy: Advocate on behalf of clients to obtain necessary services and support.
  • Collaboration: Interface with various agencies and systems to ensure seamless service delivery.
  • Case Documentation: Maintain accurate and up-to-date records of client interactions and progress.
  • Communication: Maintain effective communication with direct service providers, vocational service providers, and other stakeholders.
Requirements
  • Education: Bachelor's degree in a related field (e.g., social work, psychology, counseling).
  • Experience: Minimum 2 years of experience in case management, crisis intervention, or a related field.
  • Skills: Excellent communication and interpersonal skills, ability to work in a fast-paced environment, and proficiency in computer skills.
