About the Role

Hartford HealthCare (HHC) is seeking a highly skilled Cancer Care Specialist to join our team in the East Region. As a key member of our oncology team, you will provide value-based care to patients in practice-based settings.

Key Responsibilities:

  • Provide triage and outpatient management for cancer patients, including chemotherapy education, sick/urgent visits, and ongoing survivorship care.
  • Collaborate with healthcare teams to develop comprehensive care plans and ensure seamless patient transitions.
  • Participate in building a comprehensive cancer program and contribute to the development of evidence-based practices.

Requirements

To be successful in this role, you must possess:

  • Eligibility to be licensed in Connecticut as a Physician Assistant (PA), or Advanced Practice Registered Nurse (APRN).
  • Certification as a Physician Assistant (PA), or National Board Certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult Gerontology Primary Care Nurse Practitioner (AGPCNP).
  • A Chemotherapy Biotherapy provider card.
  • Oncology experience as a PA, NP, or RN is preferred.
