Job Summary:

Central Wyoming Hospice and Transitions is seeking a dedicated and compassionate Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families in our community.

Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and other treatments as needed.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct regular assessments and evaluations to ensure patient needs are met and care plans are effective.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements:

  • Current Wyoming Nursing license
  • 1-2 years of RN experience (preferably in hospice care)
  • Full-time RN in Converse and Platte County, willing to travel within the area
