Affinity Hospice is seeking a skilled Palliative Care Nurse Practitioner to join our team. As a key member of our medical team, you will work collaboratively with physicians to deliver high-quality care to patients in various settings.
Responsibilities- Initiate palliative care consultations or appropriate hospice treatment plans for patients in all settings.
- Conduct face-to-face encounters with hospice patients to assess their needs and develop personalized care plans.
- Provide patient education, counseling, and referrals to other healthcare professionals and community resources as needed.
- Collaborate with medical colleagues to ensure seamless patient care and communicate effectively through our electronic medical record system.
- Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving outcomes.
- Provide training and continuing education for staff members to enhance their knowledge and skills.
- Meet department productivity standards for patient visits and maintain a high level of professionalism in all interactions.
- Current unrestricted license to practice as a registered professional nurse in the state of employment.
- Graduate of an approved Nurse Practitioner program.
- Current unrestricted license to practice as a Nurse Practitioner by the Board of Nursing in the state of employment.
- Current and continued CPR certification.
- Certified Hospice and Palliative Care Nurse (CHPN) preferred.
- Minimum of 2 years of palliative care and/or hospice experience required.
- Excellent observation, verbal, and written communication skills.
- Ability to handle multiple tasks simultaneously and use critical thinking skills.
- Ability to manage emotional stresses effectively.
- Valid driver's license and reliable, insured automobile for work travel.
- Ability to bend, lift, and stoop; ability to lift 25 pounds.
- Ability to pass a background check in the state of employment.
