Role Summary:
The PRN RN/LPN at Elite Home Care Day Centers & Transportation is tasked with delivering exceptional care to members on an as-needed basis. This role involves collaborating with the day center team to enhance the physical, emotional, and mental health of our members. The position is flexible, operating Monday through Friday, across various locations in the Upstate South Carolina area.
Key Responsibilities:
- Administer medications and nursing treatments as per medical directives.
- Continuously monitor the health status of members and report any changes or concerns to the Administrator.
- Provide education to members regarding their health conditions, treatment options, and preventive care strategies.
- Maintain accurate and current member records, developing and implementing care plans as necessary.
- Review and update care plans at least biannually, along with quarterly nursing assessments for each member.
Qualifications:
- Must possess a valid and current registered nursing license in South Carolina, in good standing.
- A minimum of one year of nursing experience is required, ideally with senior adults or individuals with disabilities in a community setting.
- Strong interpersonal and communication skills are essential for effective interaction with members, families, and colleagues.
- Excellent organizational and time management abilities are necessary.
- Capability to work both independently and collaboratively within a team environment.
- Willingness to complete any required background checks and drug screenings.
