We are seeking a skilled and compassionate Registered Nurse (RN) to join our team in providing high-quality care to patients in the comfort of their own homes. As a Home Health Care Nurse, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Job Responsibilities:
  • Provide and document skilled nursing care in accordance with the developed care plan and physicians' orders
  • Deliver and coordinate quality patient care in compliance with physician orders
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner
  • Develop, implement, and update the nursing care plan
Requirements:
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
  • Minimum six months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience preferred
