Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled and compassionate Home Health Physical Therapist to join our team in Newton, Massachusetts.

About the Role:

We are looking for a dedicated and experienced Physical Therapist to provide high-quality rehabilitative care to geriatric patients in Senior Living Communities. As a Home Health Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team, working closely with nurses and other healthcare professionals to provide comprehensive care to our clients.

Responsibilities:
  • Conduct diagnostic tests and measurements to assess patients' mobility, strength, and balance
  • Develop and implement individualized care plans to promote patients' independence and quality of life
  • Provide manual therapeutic exercises, gait training, and other interventions to improve patients' functional abilities
  • Collaborate with nurses and other healthcare professionals to revise and update care plans as needed
  • Document observations, interventions, and evaluations to ensure accurate and comprehensive care management

Requirements:

To be considered for this role, you must have:

  • A degree in Physical Therapy from an accredited program
  • Current licensure as a Physical Therapist in the state of Massachusetts
  • Experience working with geriatric patients in a home health care setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
