We are seeking a skilled LCSW Behavioral Health Consultant to join our team at Community Health Net. As a key member of our primary care team, you will provide consultation to patients' medical, dental, and psychiatric providers, supporting and coordinating their mental and physical healthcare.
Key Responsibilities- Perform Behavioral Health consultation for primary care providers, utilizing evidence-based techniques to provide brief behavioral interventions.
- Conduct screenings and assessments of Behavioral Health/Substance Abuse patients, coordinating care and referring to external providers as needed.
- Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
- Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments, as clinically indicated.
- Collaborate with patients' medical providers to develop individualized treatment plans.
- Participate in regularly scheduled caseload consultation with the psychiatric consultant, communicating resulting treatment recommendations to the patient's medical provider.
- Triage patients to determine where their needs can be best met.
- Document and update patient charts in electronic health records, adhering to Community Health Net Clinical Policies and Procedures.
- Meet productivity and quality standards set forth by Community Health Net.
- Master's Degree in Healthcare, Human Services, or a related field.
- Valid and current Pennsylvania (PA) license as an LCSW, LPC, or LMFT.
- Minimum of one (1) year of work experience in a behavioral health or social service setting.
- Desirable qualifications include experience working with underserved patient populations and safety net providers, as well as familiarity with Pennsylvania and Federal programs.
