Position Overview: Licensed Practical Nurse - This role involves taking charge of comprehensive resident care within a designated unit during each shift. The Licensed Practical Nurse (LPN) is responsible for executing professional nursing tasks while ensuring that nurse aide responsibilities are fulfilled. Collaboration with various healthcare disciplines is essential to guarantee exceptional resident care.
Key Responsibilities:
- Oversee the nursing floor in partnership with the Director of Resident Services (DRS) to ensure high-quality care for residents.
- Develop assignments for nurse aides as necessary to address resident needs.
- Alert the DRS regarding any changes in resident conditions or staff-related issues.
- Administer medications and treatments following established protocols.
- Ensure the medicine cart is secured when not in direct view.
- Regularly update resident support plans as required.
- Conduct thorough assessments of residents and adhere to established procedures.
- Assist residents with activities of daily living (ADLs) as needed.
- Support residents during meal times to promote proper nutrition.
- Accompany physicians during resident evaluations to maintain continuity of care.
- Timely transcription of physician orders in accordance with policy.
- Monitor nursing care and engage with residents throughout the shift to ensure adherence to care plans.
- Participate in shift handovers to ensure seamless communication and care continuity.
- Supervise the care provided by nurse aides.
- Complete incident reports promptly in accordance with policy.
- Collect necessary samples from residents for laboratory testing.
- Communicate essential information with families, residents, and staff as required.
- Ensure adequate supplies are available on the nursing floor for all staff.
- Engage in staff meetings to foster communication across all shifts.
- Contribute to problem-solving regarding staff and operational issues.
- Assist in the orientation and training of new personnel.
- Maintain a clean, safe, and organized environment for residents, staff, and visitors.
- Document all care provided during the shift accurately and promptly.
- Chart medications immediately after administration, adhering to policy.
- Provide feedback to the DRS regarding nurse aide performance.
- Uphold confidentiality at all times.
- Participate in mandatory training, in-services, and continuing education opportunities.
- Operate within the guidelines of the Pennsylvania State Professional Standards Title 49 Chapter and the scope of practice allowed by licensure.
- Perform additional duties as assigned.
Qualifications:
- Graduation from an accredited nursing program and possession of a current, valid Practical Nurse license in good standing from the Commonwealth of Pennsylvania. A temporary practice permit is required for nurses awaiting licensure.
- Strong management, communication, and observational skills, with a focus on geriatric care.
- Proficiency in computer use, including Microsoft Word.
