Home Health Registered Nurse Case Manager

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse Case Manager. As a key member of our healthcare team, you will provide high-quality patient care and coordinate services to ensure the best possible outcomes for our patients.
Responsibilities- Provide skilled nursing services to patients in their homes, including wound care, medication management, and education on disease management.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Conduct thorough assessments of patients' medical and social needs, and develop plans to address any gaps in care.
- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Document all patient interactions, assessments, and care plans in a timely and accurate manner.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in home health care.
- Current RN licensure in the state of Washington.
- Minimum of 1 year of experience in home health care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a focus on providing high-quality patient care.
- Basic computer skills and proficiency in electronic medical records.
