Full-Time Home Health Registered Nurse Case Manager

We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team at Signature Healthcare at Home. As a key member of our home health team, you will provide high-quality patient care and manage patient care services in a timely and efficient manner.
Responsibilities- Provide skilled services to patients at home and manage patient care services pertaining to the curative, rehabilitative, and/or preventive aspects of nursing.
- Develop a care plan that establishes interventions and goals, including the patient and family/caregiver in the planning process.
- Work with intake, clinical manager, and/or DPS to coordinate patient start of care in a timely manner.
- Educate patient/caregiver regarding regulatory components for home health admission.
- Obtain consents for home health services.
- Complete an initial comprehensive assessment of patient and caregiver/living situation to determine care needs.
- Complete a comprehensive medication review and reconciliation.
- Set up an individual emergency preparedness plan.
- Establish the initial individualized plan of care in collaboration with the certifying medical provider based on comprehensive assessment and patient goals.
- Attend required staff meetings, interdisciplinary team meetings, and care coordination activities.
- Participate in on-call duties as defined by the on-call policy.
- Ensure that arrangements for equipment and other necessary items and services are available.
- Supervise ancillary personnel and delegate responsibilities when required.
- Coordinate patient care in tandem with Case Manager as assigned by Clinical Manager.
- Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Fulfill the obligation of requested and/or accepted case assignments.
- Actively participate in quality assessment performance improvement teams and activities or other committees as assigned.
To be successful in this role, you will need to have a strong background in nursing and excellent communication and interpersonal skills. You will also need to be able to work independently and as part of a team, and be able to adapt to changing situations and priorities.
