Job Summary

We are seeking a skilled Long Term Acute Care LPN to provide high-quality patient care in a dynamic and supportive environment. This role requires a strong commitment to delivering exceptional patient outcomes and working collaboratively with the healthcare team.

About the Role

The successful candidate will be responsible for delivering direct and indirect patient care to medically complex patients under the clinical supervision of a Registered Nurse. Key duties include:

  • Providing direct and indirect patient care in accordance with facility policies and procedures.
  • Monitoring and documenting patient status and condition, including vital signs, drains, dressings, and lab values.
  • Informing the Registered Nurse or Physician of activities performed, observations, and changes in patient status.
  • Administering medications according to facility policy and documenting administration.
  • Maintaining confidentiality at all times and providing a safe, comfortable, and therapeutic patient environment.
Requirements

To be successful in this role, you will need:

  • A current nursing license for the state in which you practice.
  • At least one year of experience in long-term acute care or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Computer proficiency and the ability to navigate electronic medical records.
  • A willingness to learn and adapt to new procedures and technologies.
