Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a RN Case Manager, you will be responsible for providing high-quality patient care and coordinating services to meet the unique needs of our patients.

Responsibilities
  • Provide comprehensive patient assessments and develop individualized care plans to meet the needs of patients and their families.
  • Coordinate services with healthcare providers, family members, and other stakeholders to ensure seamless care delivery.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.
  • Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Ability to travel to all business locations.
