Nurse Practitioner

Monogram Health is seeking a skilled Nurse Practitioner to join our team as an Advanced Practitioner. As a key member of our integrated Care Team, you will play a vital role in delivering personalized, compassionate medical care to patients with chronic kidney disease and related metabolic disorders.
Responsibilities- Conduct comprehensive annual wellness exams and provide counseling and education to patients and families about benefits and programs available to help them live healthier lives.
- Document patient information, including chief complaint, diagnosis, past medical and family history, review of systems, examinations, medications, allergies, assessment, and plan.
- Coordinate care with primary care providers, specialists, and ancillary services to ensure seamless patient care.
- Prescribe medications, order tests, and collaborate with patients' Monogram physicians to deliver evidence-based, timely care.
- Participate in integrated care team meetings and contribute to the development of treatment plans that prioritize patient health and satisfaction.
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Ability to work autonomously and practice without direct supervision.
- Access to transportation, a valid driver's license, and car insurance.
- Proficiency with medical instruments and equipment required by the work.
- Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology.
- Ability to communicate effectively in verbal and written form with patients, family members, physicians, and representatives of the community.
- Sound understanding of all federal and state regulations, including HIPAA and OSHA.
