About the Position

We are seeking a skilled Medical Care Provider to join our team at Abode Care Partners.

Job Summary

The successful candidate will provide compassionate, evidence-based medical care to seniors and patients with complex needs. This role involves examining and treating chronic illnesses, ordering diagnostic tests, and collaborating with other healthcare professionals to ensure comprehensive care.

Responsibilities:
  • Performing routine check-ups and providing preventative care
  • Treating chronic illnesses, such as diabetes and hypertension
  • Ordering diagnostic tests, including laboratory and imaging studies
  • Collaborating with caregivers and other healthcare professionals to develop and implement individualized care plans
  • Ensuring patient medical records are accurate and up-to-date
Requirements:
  • A Master's degree in Nursing (MSN)
  • An active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • At least two years of nurse practitioner experience
