About the Role:

We are seeking a skilled Primary Care Physician to join our team at AbsoluteCare Inc. As a Primary Care Physician, you will be responsible for providing high-quality medical care to patients with complex and chronic medical conditions.

Key Responsibilities:

  • Manage and improve health outcomes for patients with multiple complex and chronic medical conditions.
  • Lead and collaborate with a multidisciplinary team, including social work, case management, behavioral health, pharmacy, nutrition, and other healthcare professionals.
  • Develop transitional care plans and track patients' progress along the continuum of care.
  • Communicate and interact with alternate levels of care facilities, such as hospitals, ERs, skilled nursing facilities, and home care.
  • Treat acute exacerbations of chronic conditions within the AbsoluteCare center or refer patients to higher levels of care when necessary.
  • Accept same-day or next-day appointments for urgent cases and provide intensified physician or case management services.
  • Maintain vigilance in preventing ER visits, hospital admissions, and readmissions by keeping track of patients' disease processes.
  • Educate members on chronic disease progression and advance directives where applicable.
  • Practice evidence-based medicine in accordance with the PCMH model of care.

Candidate Qualifications:

  • Licensed Physician in the state of practice.
  • Board certified in Internal Medicine or Family Practice.
  • BLS certification required.
  • Mission-driven to serve the underserved.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Experience using EMR systems.
  • Problem-solving skills and ability to think creatively.

Preferred Qualifications:

  • Experience in population health management.
  • Experience working with psycho-socially and medically complex patient populations.
