We are seeking a highly skilled Psychiatric Social Worker II to join our team at Heluna Health. As a key member of our mental health services team, you will provide professional services to clients in need of health and mental health services.
Key Responsibilities- Conduct comprehensive assessments of clients' psychosocial and service needs through interviews, evaluations, and reviews of medical and mental health records.
- Develop and implement care management plans to address clients' mental health and substance use needs.
- Collaborate with other agencies and community providers to ensure seamless coordination of care and services.
- Provide education and support to clients and their families on mental health and wellness.
- Participate in team conferences and staff meetings to stay up-to-date on new developments and best practices.
- Master's degree in Social Work from a Council on Social Work Education-accredited graduate school.
- Minimum of two years of clinical social work experience providing individual, group, or family psychotherapy, assessment, and treatment planning.
- Valid and active license as a Licensed Clinical Social Worker issued by the California Department of Consumer Affairs, Board of Behavioral Sciences.
- Valid California Class C Driver License or ability to utilize alternative transportation methods.
This is a full-time position in a general office setting with a temperature-controlled environment.
