Key Responsibilities
  • Conduct thorough assessments of patients' physical, psychological, social, and spiritual needs to develop and implement effective care plans.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions.
  • Provide education and support to patients and their families to promote health and wellness.
  • Develop and maintain accurate and comprehensive patient records, charts, and progress notes.
  • Perform therapeutic, preventative, and rehabilitative nursing procedures as directed by attending physicians.
Requirements
  • Graduate from an accredited registered nursing program and licensed in the state.
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation, or medical surgical nursing.
  • Home health experience preferred.
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license, and appropriate automobile insurance.
