Position Summary
The Care Coordination Nurse plays a vital role in the Population Health and Health Services sector. This position involves a collaborative approach to assess, plan, implement, monitor, and evaluate the healthcare needs of members, providers, and communities.
Key Responsibilities
1. **Assessment**: Conduct thorough evaluations of members' situations to identify healthcare needs and develop comprehensive management plans.

3. **Implementation**: Execute targeted interventions to achieve established goals.
4. **Coordination**: Organize and integrate necessary resources to fulfill care objectives.

Qualifications
Education: High School Diploma or equivalent is required.
Experience: A minimum of 4 years in direct patient care is essential, along with knowledge of managed care and medical terminology.
Skills: Strong analytical abilities, effective communication skills, and a detail-oriented approach are necessary for success in this role.
Licensure and Certification
Current Louisiana licensure as a Licensed Practical Nurse (LPN) is mandatory. Additional certifications in Disease Management or Health Care Management, as well as Utilization Review or Managed Care, are preferred.
Work Environment
