Job Title: C&F Case Manager

Lincoln Behavioral Services is seeking a highly skilled and compassionate C&F Case Manager to join our team. As a C&F Case Manager, you will provide community-based support and advocacy to families of children who are mentally or emotionally disturbed, and who are at risk of hospitalization, or vulnerable individuals.

Key Responsibilities:
  • Complete comprehensive assessments for all clients referred for case management services.
  • Develop and implement individualized service plans to address client needs.
  • Provide individual and family planning and review as related to client needs.
  • Transport consumers to community appointments, clinic appointments, activities, and other resources as identified in their person-centered plan.
  • Participate in clinical team meetings and coordinate team interventions.
  • Conduct case management services and provide crisis intervention effectively.
  • Maintain accurate and timely documentation according to agency standards.
Requirements:
  • Bachelor's degree in social work from an accredited college or university required.
  • Preferred 2 years of experience in working with mentally or emotionally disturbed children.
  • Credentialing, licensure - Nursing, Social Work, Counseling, or Psychology License with the State of Michigan is required.
