Job Summary

We are seeking a skilled and experienced RN MDS Nurse to join our team at SNF Rehab & Healthcare. As a key member of our healthcare team, you will be responsible for coordinating the Resident Assessment Process and ensuring the accurate and timely completion of Minimum Data Sets (MDS) and Care Plans.

Key Responsibilities
  • Resident Assessment Process: Direct the Resident Assessment Process through assisting with the completion of the Minimum Data Set (MDS) and CAA's.
  • Care Plan Development: Participate in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes as required by Federal and State regulations.
  • MDS Completion: Complete nursing assigned MDS items, CAA's and Care Plans as designated by the facility.
  • Collaboration and Communication: Participate in the resident Care Plan Meetings, Utilization Review Meetings & Triple Check.
Requirements
  • Licensure: RN licensure in the state of NJ.
  • Knowledge and Skills: Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care. Computer skills-Windows applications, computer experience in Point Click Care (PCC) preferred.
  • Experience: Previous experience in completing MDS and Care Plans is required. Previous experience in a long-term facility, 1-3 years of MDS experience or RN with LTC experience that can be trained in assessment role.
  • Personal Qualities: Can work independently, flexible with schedule for month end close, detail oriented and able to accurately and timely complete assessments.
