Job Summary:

We are seeking a skilled RN to join our team as a Weekend Call Triage Nurse. In this role, you will be responsible for triaging patient calls during non-business hours, providing outstanding nursing, palliative care, and bereavement support.

Key Responsibilities:

  • Triage patient calls during non-business hours, utilizing critical thinking skills to determine the priority of patient needs and whether a home visit is necessary.
  • Provide emotional support to caregivers and make required notifications during all deaths.
  • Document all calls in the patient's electronic medical record, ensuring accurate and timely documentation.
  • Collaborate with the interdisciplinary team to ensure quality and safe delivery of care, respecting client confidentiality in all communication.

Requirements:

  • Current licensure in good standing as a Registered Nurse in the State of Illinois.
  • 1-3 years of experience in/with recent hospice/home health care nursing background.
  • Ability to communicate effectively with patients, caregivers, and the interdisciplinary team.
