Job Title: Home Care Registered Nurse

Liberty Health System is seeking an experienced Home Care Registered Nurse to join our team.

About the Role:

As a Home Care Registered Nurse, you will be responsible for providing professional nursing services to patients in their homes. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that promote favorable outcomes and independence for our patients.

Key Responsibilities:
  • Participate in the interdisciplinary care planning process to ensure comprehensive and coordinated care.
  • Provide professional nursing services in accordance with established care plans, promoting patient safety and well-being.
  • Document patient information accurately and thoroughly to facilitate interdisciplinary coordination and quality improvement activities.
  • Demonstrate effective communication and interpersonal skills to build strong relationships with patients, families, and healthcare team members.
  • Stay up-to-date with industry developments and best practices to provide high-quality care and services.
Requirements:
  • Current RN licensure in the state where the branch is located.
  • At least one year of clinical nursing experience in an acute care or long-term setting.
  • Home health experience preferred but not required.
  • Excellent clinical nursing skills and knowledge of fundamental medical-surgical nursing principles and practice.
  • Ability to communicate effectively, both verbally and in writing.
  • Proficiency in computer applications and ability to use a computer for patient documentation and communication.
