Job Title: Home Care Registered Nurse

Liberty Cares is seeking an experienced Home Care Registered Nurse to join our team. As a Home Care Registered Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Job Summary:

We are looking for a skilled and compassionate Registered Nurse to provide professional nursing services to our patients. The ideal candidate will have a strong background in medical-surgical nursing and experience working in home health settings.

Responsibilities:
  • Participate in the interdisciplinary care planning process to ensure comprehensive patient care.
  • Provide professional nursing services in accordance with the plan of care established by the Patient Care Coordinator.
  • Document patient information accurately and thoroughly to facilitate interdisciplinary coordination of services and quality improvement activities.
  • Demonstrate positive and effective interpersonal relationships with patients, families, and healthcare team members.
  • Stay up-to-date with the latest medical-surgical nursing principles and practices.
Requirements:
  • Current RN licensure in the state where the branch is located.
  • At least one year of clinical nursing experience in an acute care or long-term setting.
  • Home health experience preferred but not required.
  • Excellent clinical nursing skills and knowledge of fundamental medical-surgical nursing principles.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to use a computer and have a working knowledge of various computer applications.
