Job Summary

We are seeking a skilled Registered Nurse Case Manager to join our Home Health team in the Winfield and Arkansas City, KS areas. As a PRN Home Health Registered Nurse Case Manager, you will play a vital role in delivering high-quality patient care and collaborating with our interdisciplinary team to achieve exceptional patient outcomes.

Responsibilities
  • Assist in the development, review, and revision of patient care plans to ensure comprehensive and individualized care.
  • Provide nursing care in accordance with physician orders, individualized patient care plans, and authorized payer sources.
  • Observe, record, and report patient reactions to treatment and any changes in condition to appropriate personnel and/or physicians.
  • Provide ongoing instruction and supervision to patients, families, or other healthcare team members in the plan of treatment.
  • Participate in education programs, quality assessment activities, nurse meetings, and other staff meetings as required.
Requirements
  • Kansas or compact nursing license.
  • Strong organizational and self-management skills.
  • Valid driver's license.
