Job Title: Direct Care Worker

At Change Incorporated, we are seeking a compassionate and dedicated Direct Care Worker to join our team. As a Direct Care Worker, you will play a vital role in providing supportive services to individuals in a home and community-based setting.

Key Responsibilities:

  • Provide conversational support and companionship to individuals, promoting a sense of belonging and connection.
  • Establish and maintain a helpful and supportive relationship with each individual, fostering trust and open communication.
  • Teach and assist individuals to accomplish recovery plan goals, promoting independence and self-advocacy.
  • Communicate daily progress reports to immediate supervisors and other agency representatives, ensuring seamless collaboration and support.
  • Accompany individuals to medical/health appointments or social activities, providing transportation and support as needed.
  • Perform light housekeeping tasks to maintain a safe and comfortable living environment.

Preferred Qualifications:

  • Experience in the healthcare field providing direct care or strong customer service skills.
  • Reliable transportation, a valid driver's license, and current automobile insurance.
