Peer Case Manager

We are seeking a highly motivated and compassionate Peer Case Manager to join our team at Special Service for Groups, Inc. - HOPICS. As a Peer Case Manager, you will play a critical role in providing professional case management services to homeless consumers, linking them with essential resources, and supporting them in their journey towards recovery and stability.
Key Responsibilities- Conduct comprehensive needs assessments with consumers to identify their immediate needs and develop personalized treatment plans.
- Provide case management services, including linking consumers with housing, employment, education, and healthcare resources.
- Collaborate with a multidisciplinary team to ensure comprehensive care and support for consumers.
- Document all interactions and progress in client files, ensuring accurate and timely reporting.
- Participate in regular supervision, training, and meetings to enhance skills and stay up-to-date on best practices.
- High school diploma or equivalent required; Registered Alcohol and Drug Abuse Counselor (RADAC) certification preferred.
- Lived experience with homelessness and experience working with homeless populations.
- Ability to work with diverse cultural, ethnic, and socio-economic backgrounds.
- Effective communication and interpersonal skills.
- Ability to work in unpredictable situations, crises, and high-stress environments.
This position requires working in various environments, including homeless encampments, and interacting with individuals with severe physical and mental disabilities. The incumbent must be able to work effectively in high-crime areas, deal with traumas, and maintain professional boundaries.
This is a full-time position, requiring regular attendance, punctuality, and a neat and professional appearance. The incumbent must be able to work effectively under pressure, process information quickly, and think critically.
