The Registered Nurse Care Navigator is a key member of our healthcare team, responsible for coordinating and delivering patient care utilizing the Nursing Process within the standards, policies, procedures, and guidelines of Trinity Health.
Key Responsibilities- Provides daily care coordination and nursing care to patients with one or more chronic diseases, promoting evidence-based practice and quality initiatives.
- Assists physicians with patient assessment, examinations, and treatments, ensuring seamless care coordination.
- Conducts pre-visit patient assessments, documents patient care observations, and develops patient plans of care, integrating NCQA PCMH and other accrediting body requirements.
- Educates and explains procedures to patients and family members, assisting them in coping with illnesses.
- Graduate from a credited School of Nursing
- Current License to practice as a Registered Nurse in the State of Ohio
- Minimum of two years clinical nursing inpatient hospital experience
- Two years ambulatory setting preferred
- One year Case Management experience preferred
