As a Senior Patient Care Specialist, you will play a vital role in delivering compassionate, patient-centered care to those who need it most. This full-time position involves working closely with Registered Nurses, Social Workers, and other interdisciplinary team members to ensure the highest quality of care.

Compensation Package:

  • Starting salary: $18.00 per hour (plus experience-based increments)
  • Benefits package including health insurance, retirement plan, and paid time off
  • Bonus eligibility: up to $3,000 annually

Job Responsibilities:

Key Functions:
  1. Assist patients with activities of daily living, such as bathing, dressing, and grooming
  2. Provide emotional support and companionship to patients and their families
  3. Collaborate with healthcare teams to develop and implement personalized care plans
  4. Document patient progress and report significant changes or incidents to the interdisciplinary team
  5. Maintain confidentiality and adhere to HIPAA guidelines

Requirements:

  • High school diploma or equivalent required; certification in nursing assistance an asset
  • 1 year of personal care experience, preferably in hospice or home health
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Ability to work effectively in a fast-paced environment and maintain professional boundaries
