Job Opportunity:

We are seeking a skilled Nurse Practitioner to join our team at Personalized Health Partners. As a key member of our primary care team, you will be responsible for delivering high-quality patient care in a collaborative and dynamic environment.

About the Role:
  • Provide Comprehensive Patient Care: Assess, diagnose, and manage chronic and acute conditions, providing health education and developing treatment plans tailored to each patient's needs.
  • Collaborate with Interdisciplinary Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and achieve optimal patient outcomes.
  • Foster Strong Relationships: Develop and maintain excellent relationships with patients, families, and caregivers, ensuring personalized care and support throughout the patient journey.
Requirements:
  • Education and Certification: Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP), along with national certification as an Advanced Practice Registered Nurse (APRN).
  • Licensure and Experience: Active, unrestricted state license, DEA registration, and a minimum of 2 years' experience as a Nurse Practitioner in adult or gerontological care settings.
  • Skillset: Excellent communication, clinical assessment, and time management skills, with the ability to work independently and collaboratively within a fast-paced environment.
Preferred Qualifications:
  • Gerontological Expertise: Training or experience in geriatrics, with a passion for caring for senior populations.
  • Leadership Experience: Proven track record of leadership and collaboration in value-based care models, with a focus on driving positive patient outcomes.
Compensation and Benefits:

We offer a competitive salary range of $120,000 - $150,000 annually, based on experience, as well as a comprehensive benefits package, including medical, dental, vision, and 401(k) retirement plan with employer match.
