Job Responsibilities:
  • The Hospice Care RN Case Manager is responsible for planning, organizing, and directing hospice services, with a strong focus on community health education and experience.
  • Conducts initial, comprehensive, and ongoing assessments of patients and their families to identify hospice needs, providing thorough physical assessments and medical histories.
  • Delivers professional nursing care by employing all elements of the nursing process.
  • Evaluates patient status through:
    • Creating and initiating a care plan
    • Regularly reassessing patient and family needs
    • Updating the care plan as necessary
  • Utilizes health assessment data to formulate nursing diagnoses.
  • Develops care plans that set achievable goals based on nursing diagnoses, incorporating palliative care actions and involving patients and families in the planning process.
  • Implements appropriate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed.
  • Provides counseling to patients and families to address nursing and related needs.
  • Offers health care instructions tailored to the patient’s assessment and care plan.
  • Assists patients with daily living activities, promoting self-sufficiency and comfort care.
  • Acts as the RN Case Manager as assigned, coordinating care for an assigned patient caseload.
  • Participates in on-call duties as outlined in the on-call policy.
  • Ensures availability of necessary equipment and services.
  • Supervises ancillary staff and delegates responsibilities as needed.
  • Commits to personal growth and development, enhancing professional knowledge through continuing education and in-service training.
  • Engages actively in quality assessment and performance improvement initiatives.
  • Maintains accurate clinical documentation regarding patient conditions and care provided.
  • Communicates effectively with physicians regarding patient needs and condition changes, ensuring timely updates and care coordination.
  • Educates patients and families on self-care techniques and provides necessary instructions as directed by physicians.
  • Creates a safe environment for patients and collaborates with families and interdisciplinary teams to meet emotional and physical needs.
  • Attends interdisciplinary meetings to ensure comprehensive care delivery.

Qualifications:
  • A genuine passion for hospice care and the ability to build meaningful relationships with patients and their families.
  • Ability to work independently with minimal supervision.
  • A valid driver's license and a reliable, insured vehicle.
  • 1-2 years of recent acute care experience in a healthcare setting (required), with home care experience preferred.
  • Graduation from an accredited nursing program.
  • Understanding of hospice philosophy and comfort in addressing issues related to death and dying.
  • Empathetic approach towards the needs of patients and families.
  • Maintains a calm demeanor in patient homes.

Combination of facility and home-based hospice care, managing an average caseload of 12-15 patients. Utilizes Homecare Homebase and nVoq voice app for documentation, with all necessary equipment provided.
