Hospice Nurse

Sage Group HealthCare Staffing is seeking a skilled RN Hospice to provide high-quality care to patients in the comfort of their own homes. As a Hospice Nurse, you will be responsible for developing and implementing individualized care plans, providing emotional support to patients and their families, and ensuring seamless coordination of care.
Key Responsibilities:
- Provide comprehensive nursing care to patients with terminal illnesses, including pain management and symptom control.
- Conduct thorough assessments of patients' physical and emotional needs, and develop personalized care plans to address these needs.
- Collaborate with patients, families, and healthcare teams to ensure effective communication and coordination of care.
- Provide education and support to patients and their families on disease management, treatment options, and end-of-life care.
- Document patient progress, including medical history, treatment plans, and outcomes.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
Requirements:
- Registered Nurse (RN) license in the state of Wisconsin or compact state.
- 1 year of experience in home health or hospice care.
- Recent/Current Home Health Nursing experience: 1 year within the last 3 years – Required.
- Recent/Current Hospice Nursing experience: 1 year with the last 3 years – Required.
- Valid Driver's License and Personal Auto Insurance.
- Covid Vaccination or medical/religious exemption.

- Competitive pay rate: $2000-$2080 weekly gross pay.
- Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers.
- Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments.
