Position Summary:
The Client Support Specialist will assess the resources necessary to fulfill the unique requirements of each participant. This role involves taking all essential actions to ensure the execution of the service treatment plan, acting as an advocate for participants to facilitate access to required services, identifying program needs, recognizing service gaps, and proposing effective strategies. A strong commitment to the values of Resources for Human Development is expected throughout the performance of job responsibilities.
Key Responsibilities:
Collaborative Planning
- Participate as a member of a multidisciplinary team aimed at delivering services from a holistic and values-driven perspective.
- Contribute to the creation and evaluation of recovery plans in partnership with program participants, clinical staff, and other relevant stakeholders.
- Establish and nurture relationships with community organizations to assist program participants in accessing available resources.
- Provide community-oriented services to program participants, emphasizing education, resource connection, community engagement, and the achievement of recovery plan objectives.
- Complete all necessary assessments and documentation as mandated by Resources for Human Development, funding sources, and licensing/accreditation bodies.
- Perform additional responsibilities as assigned.
- Bachelor's Degree in Human Services or a related field.
- Intermediate proficiency in computer skills.
- Preferred: One year of experience in Case Management.
- Required: Two years of general work experience supporting individuals in behavioral health, addiction recovery, or intellectual/developmental disabilities services.
- Valid driver's license.
- Engagement in community-based activities within the service delivery area of the program, including transportation of program participants to appointments.
- Medium lifting: up to 50 pounds occasionally, up to 20 pounds frequently, and up to 10 pounds constantly.
