PRIMARY RESPONSIBILITIES:

As a Practical Nursing Specialist, you will oversee a designated group of patients under the guidance of a registered nurse or physician. You will collaborate within a patient-focused care team dedicated to delivering comprehensive care tailored to individual patient needs, enhancing their overall health and wellness. Reporting directly to the Director of Nursing and Clinical Services, your role will be pivotal in providing exceptional healthcare services.

KEY DUTIES:
  • Support patient care initiatives under the supervision of a Registered Nurse or Provider
  • Efficiently gather and evaluate patient information, addressing their physical, emotional, and cognitive requirements
  • Prioritize patient care and promptly communicate any changes in patient condition to the RN/Provider
  • Accurately implement and adhere to established care plans
  • Deliver age-appropriate care and assistance to patients
  • Work collaboratively with the care team to ensure patients receive essential education, referrals, medications, and preventive services
  • Guide patients and families on self-care strategies and health-enhancing practices
  • Maintain precise and timely records in the electronic medical record (EMR) system
  • Engage in care team discussions and quality improvement initiatives
  • Organize follow-up appointments and tests as directed by the provider
  • Assist with various administrative tasks, including inventory control and scheduling
  • Uphold safety protocols and legal standards in patient care
  • Conduct diagnostic tests and procedures as necessary
  • Administer medications and vaccines with accuracy
  • Comply with infection control protocols consistently
  • Emphasize patient-centered care and strive for patient satisfaction
REQUIRED QUALIFICATIONS:
  • Completion of an accredited LPN program with valid state licensure
  • Prior experience in a clinical office environment is preferred
  • Excellent communication and organizational abilities
  • Basic Life Support (BLS) certification is mandatory
PHYSICAL DEMANDS:

This position requires standing for extended periods, along with some lifting and bending tasks.
