Job Overview:
Shriners Children's Lexington is seeking a RN Care Manager to enhance our healthcare team.


The RN Care Manager plays a crucial role in conducting assessments, planning interventions, and providing follow-up care for patients and their families at Shriners Children's throughout the entire care continuum.

By becoming part of our team, you will have the chance to positively influence our patients' experiences. You will assist in ensuring continuity of care that leads to favorable outcomes in our family-centered environment.

Work schedule: Monday - Friday, day shift, with no weekends or holidays.

Employee parking is provided at no cost.

Responsibilities:


Facilitates the care plan to achieve targeted outcomes and integrates the efforts of the healthcare team, including the patient's Primary Care/Medical Home, by coordinating necessary resources and services to meet established goals.

Organizes services among community organizations, healthcare providers, patients, families, caregivers, and others involved in the care plan.

Ensures timely execution of interventions for optimal results and seamless transitions throughout the care continuum.

This list is not exhaustive; the incumbent may be required to perform additional related duties and engage in special projects as assigned.

Qualifications:

Required:
Current full Covid-19 vaccination status is mandatory.

Bachelor of Science in Nursing (BSN) is required.

Experience in disease management and the application of motivational interviewing techniques is essential.

Preferred:
Master of Science in Nursing (MSN) is preferred.

5+ years of clinical experience in Pediatrics, Burn care, and/or Orthopaedics is preferred.

3-5 years of experience in Care Coordination/Case Management is preferred.

Experience with utilization review/management and a working knowledge of InterQual criteria is strongly preferred.
